                  Also known as "low iron". Common causes may include poor iron in diet, virus/bacteria infections, reaction or side effect of medications, or bleeding. Common symptoms may include weakness, fatigue (tired), skin color looking pale and sometimes even shortness of breath with activity.

                • Arthritis

                  Cost: $131.00

                  Inflamed or swollen joint(s). Common symptoms may include swelling, pain, and stiffness.

                • Asthma

                  Cost: $131.00

                  May become worse with exercise, colds, bronchitis or allergies; common symptoms may include a frequent dry cough, wheezing or mild shortness of breath. NOTE: if life-threatening shortness of breath or illness call 911 or go the nearest Emergency Room.

                • Bladder or Urinary Tract Infection

                  Cost: $97.00

                  Common causes may include bacteria infection, dehydration, virus, kidney stone, sexual transmitted infection. Common symptoms may include urinating frequently, persisting urge to urinate, and pain or burning with urinating.

                • Bronchitis

                  Cost: $131.00

                  Often is inflammation of the bronchus caused by a virus or bacteria. Common symptoms include coughing, sometimes a thick mucous causes chest congestion, fatigue, fever and sometimes wheezing.

                • Cold Sores

                  Cost: $97.00

                  Often caused by a virus infection. Common symptoms may include a blister or sore on or near the lips or mouth.

                • Common Colds

                  Cost: $97.00

                  Common symptoms may include a runny or stuffy nose, mild sore throat, cough, nasal congestion, fatigue, low grade fever, ear pressure or pain, and sometimes watery eyes.

                • Diarrhea or Vomiting

                  Cost: $131.00

                  Often caused by infection from a virus/bacteria/parasite or reaction to food/medication. Common symptoms may include frequent loose or watery stools, and often times accompanied by a stomach ache.

                • Ear Aches/Problems

                  Cost: $97.00

                  Common causes may include ear aches may be a middle or outer ear infection, foreign object in the ear, fluid in the middle ear, or minor injury. Common symptoms may include ear itching, pressure, pain, ear drainage, fever, or decreased hearing.

                • Ear Wax Removal

                  Cost: $97.00

                  Often ear wax may build up in the ear canal. Common symptoms may include poor hearing, a full feeling, itching, tenderness, or irritation. To clear ear wax a therapeutic washing out of wax buildup is used and sometime may need an ear wax softener.

                • Eye Infections/Problems

                  Cost: $97.00

                  Common causes may include infection, stye, dry eyes, allergies, foreign objects causing irritation, or virus. Common symptoms may include redness, itching, irritation, pain, pressure, fever, swelling, draining/crusted (matting) or tearing, sudden problems seeing.

                • Gout

                  Cost: $131.00

                  Often from a type of arthritis or increase uric acid level. Common symptoms may include pain or swelling/inflammation of joints usually toes/feet, redness, pain with walking/standing or burning/hot feeling.

                • Headaches

                  Cost: $131.00

                  Common causes may include stress, migraines, dehydration, viruses or bacteria infections, allergies, sinus pressure. Common symptoms may include pain to head, vision changes, congestion, pounding or throbbing, fatigue (tired) or ringing/buzzing in the ears.

                • Hemorrhoids

                  Cost: $97.00

                  Swollen veins inside/outside the rectum/anus. Common causes include constipation, recent child birth or diet lacking fiber. Common symptoms may include pain with bowel movements, slight bright red blood with bowel movement, swelling/inflammation at or inside the rectum and sometimes pain with sitting or activity such as riding a bike.

                • Flu/Influenza

                  Cost: $131.00

                  Commonly known as the Influenza virus. Common symptoms of Influenza may include coughing, sneezing, runny or stuffy nose, body aches, chills, fevers, irritated or watering eyes, and congestion. Possibly to require additional lab charge.

                • Laryngitis

                  Cost: $131.00

                  Common causes may include sore throat, viruses, allergies, frequent coughing or reflux (GERD). Common symptoms may include hoarse or loss of voice, sore throat, discomfort with swallowing, runny or stuffy nose, low grade fever or tender lymph nodes.

                • Medication Side Effects

                  Cost: $131.00

                  Sometimes medications either prescribed or over the counter cause problems or interaction with your body. Common side effects of many medications may include simple allergy reactions such as rash/hives, nausea/vomiting/diarrhea, dizziness, change in mood, body aches, fatigue/drowsiness (tired), headaches, stomach ache, constipation.

                • Mononucleosis Infection

                  Cost: $131.00

                  Usually caused by the Epstein Barr Virus. Common symptoms may include sore throat, fever, swollen lymph nodes, neck pain, body aches, chills, fatigue (tired), headache and sometimes stomach ache. Possibly to require additional lab charge.

                • Genital Problem or Sexually Transmitted Diseases (STDs)

                  Cost: $131.00

                  Common causes of genital problems may include chaffing or skin irritation, allergic reaction, a bacteria/virus/yeast infection or sometimes sexually transmitted infections. Common symptoms may include, itching, irritation, pain, drainage/discharge cracked or open skin or sores, redness/rash/blisters on the genitals or surrounding skin, fever, body aches, chills, stomach pain, nausea/vomiting or bad odor. Possibly to require additional lab charges.

                • Rashes/Minor Skin Problems

                  Cost: $131.00

                  Common causes may include ringworm, chicken pox or shingles, eczema, impetigo, allergic skin reaction, virus, bacteria, yeast infection, sunburn, ingrown hair, foreign object in the skin, poison ivy/oak, psoriasis, dry skin, anxiety reaction, medication side effect, skin color changes or small bruises. Common symptoms may include itching, irritation, redness, pain, mild swelling, blisters, fever, body aches, dry or flaking skin, cracking of skin, pus or draining sores, rough patches or scaly skin.

                • Sinus Infections

                  Cost: $131.00

                  Common causes may include colds, congestion, sinus infections either bacteria or virus, foreign objects in the nose/sinuses or allergies. Common symptoms may include stuffy and/or runny nose, sinus pressure or pain, post nasal drip, dry or sometimes hacking cough, fevers, headaches.

                • Splinter/Sliver Removal

                  Cost: $97.00

                  Often a result of a minor injury to the skin after a puncture from a foreign object that becomes stuck in the skin such as a piece of glass/metal/wood/tack/nail and sometimes dirt or gravel. Common symptoms may include bleeding, redness, swelling, itching, pain or infection. Sometimes additional treatment is needed and this may require a tetanus booster vaccine, stitches, staples or skin glue.

                • Stomach Ache/Problems

                  Cost: $131.00

                  Common causes may include viruses/bacteria/yeast infections, reaction to food or medications, heartburn/acid reflux (GERD), ulcers, gas, constipation, diverticulitis, appendicitis, irritable bowel syndrome, parasites, dehydration and many more. Common symptoms may include an upset stomach/nausea, vomiting, stomach cramps or sharp pains, diarrhea, body aches, chills, fatigue (tired), fever, rash and sometimes dehydration.

                • Tonsillitis/Sore Throat

                  Cost: $131.00

                  Common causes may include an infection of bacteria/yeast/viruses, allergies, frequent coughing, mono, strep throat, post nasal drip, or acid reflux (GERD). Common symptoms may include pain with swallowing/talking, enlarged tonsils or tender lymph nodes on the neck, a change in voice or laryngitis, runny or stuffy nose, fever, headache, fatigue (tired), body aches, chills and sometimes stomach ache. Possibly to require additional lab charge.

                • Toothache

                  Cost: $97.00

                  Common causes may include tooth injury, infection, cracked tooth, jaw problems, teeth grinding, cavities, Common symptoms may include pain, trouble chewing, pressure, jaw ache, headache, fever, drainage/pus on gums/tooth/mouth, sores in mouth, and swelling in mouth/face.

                • Yeast Infections

                  Cost: $97.00

                  Commonly in the mouth (thrush), on the skin, feet/toes (athletes foot), or genitals. Common symptoms may include, itching, irritation, pain, redness or rash on the skin, and in genitals sometimes white drainage/discharge. Possibly to require additional lab charges.

                • Injuries

                  Laceration/Cuts/Wound Repair up to 2.5cm with Sutures and Multiple Layers

                  Cost: $192.00

                  A laceration requires stiches/staples/or skin glue up to 2.5cm to repair and close possibly multiple layers of skin.

                  Laceration/Cuts/Wound Repair up to 2.4cm with Sutures (simple)

                  Cost: $131.00

                  A simple laceration 2.4cm or less to repair and close skin.

                  Laceration/Cuts/Wound Repair up to 0.5cm Simple

                  Cost: $97.00

                  A simple laceration requires Dermabond or steri strips 0.5cm or less to repair and close skin.

                • Severe Sprains

                  Cost: $131 to $192 Depending on Severity

                  Common causes may include twisting (spraining) a hip/shoulder/ect, muscle that was pulled/injured or overworked. Common symptoms include swelling, tight muscles, bruises, pain/pressure that stops normal activity/ movement, can be a sharp or dull pain which sometimes shoots up or down from the injured area. Possibly to require additional x-ray charge.

                • Moderate Sprains

                  Cost: $131.00

                  Common causes may include twisting (spraining) ankles/wrists/ect, muscle that was pulled/injured or overworked. Common symptoms include swelling, tight muscles, bruises, pain/pressure that stops normal activity/ movement, can be a sharp or dull pain which sometimes shoots up or down from the injured area. Possibly to require additional x-ray charge.

                • Simple Sprains

                  Cost: $97.00

                  Common causes may include twisting (spraining) a finger/toe/ect, muscle that was pulled/injured or overworked. Common symptoms include swelling, tight muscles, bruises, pain/pressure that stops normal activity/ movement, can be a sharp or dull pain which sometimes shoots up or down from the injured area. Possibly to require additional x-ray charge.

                • Acute Fractures/Broken Bones (Simple)

                  Cost: $192.00

                  Simple fractures usually are not out of place and often occur due to a minor injury which may include a twisting of a arm/leg/elbow or from falling/jumping/landing on the area and sometimes when an object falls on the area. Common symptoms include swelling, pain/pressure that stops normal activity/movement, can be a sharp or dull pain which sometimes shoots up or down from the injured area. Possibly to require additional x-ray charge.

                • Animal Bites (Simple)

                  Cost: $131.00

                  Often simple bites are from a cat, dog, raccoon, domestic ranch animal or common pet. Common symptoms may include pain, tight muscles, pain/pressure that stops normal activity/movement, can be a sharp or dull pain which sometimes shoots up or down from the injured area.

                • Backaches/Strains (Acute, New or Sudden Onset)

                  Cost: $192.00

                  Common causes may include twisting (spraining) a arm/leg/ankle/wrist/elbow/ finger/toe, muscle that was pulled/injured or overworked. Common symptoms include swelling, tight muscles, bruises, pain/pressure that stops normal activity/movement, can be a sharp or dull pain which sometimes shoots up or down from the injured area.

                • Bruises/Cuts/Scrapes or Simple Puncture/Fish Hook Injuries

                  Cost: $97.00

                  Often a result of a minor injury to the skin causing an opening or a collection of blood under the skin, or puncture such as with an imbedded fish hook (not requiring incision for removal). Common symptoms may include bleeding, swelling, pain, and sometimes foreign objects may become stuck in the skin such as gravel, dirt, piece of glass/metal/wood and sometimes a tack/nail. Sometimes this may require a tetanus booster vaccine, stitches, staples or skin glue or removal of foreign objects. Additional costs if an incision for removal is required.

                • Burns (Minor) or Sunburns

                  Cost: $97.00

                  Minor burns are usually in a small area and superficial surface of the skin. Common symptoms may include redness, itching, tenderness/pain, blisters, flaking/peeling or dry skin.

                • Crush Injuries

                  Cost: $192.00

                  An injury by an object that causes compression to the body or a body part. Possibly to require additional x-ray charge.

                • Eye Injuries (Minor)

                  Cost: $97.00

                  Often caused by foreign objects such as dirt, grass, leaves, gravel, metal fleck, fingernail scratches, eyelash/lint irritation, splashed/spilled liquid into the eye. Common symptoms may include eye redness, itching, irritation, pain, pressure, swelling, draining/crusted (matting) or tearing eyes and sometimes sudden problems seeing.

                • Finger/Toenail Injuries or Problems

                  Cost: $97.00

                  Often a result of a minor injury to the fingernail/toenail which causes an opening or a collection of blood under the nail, a puncture with a foreign object such as a fish hook, nail, staple or glass. Common symptoms may include bleeding, swelling, pain, and sometimes foreign objects may become stuck in the nail. Sometimes this may require a tetanus booster vaccine, stitches, staples or skin glue or removal of foreign objects.

                • Insect Bites/Bee Stings

                  Cost: $97.00

                  Common symptoms may include itching, redness, swelling, hives, irritation or pain, open/blistered sore and sometimes draining pus, stingers may become stuck in the skin.

                • Tick Removal

                  Cost: $97.00

                  Removal of a tick is a simple procedure and can be assessed for complications. Ticks are common in the spring/summer/early fall seasons and sometimes bit or become imbedded in the skin.

                • Wart/Skin Tag/Mole Removal (Simple) (Each Lesion)

                  Cost: $97.00

                  Removal of a benign appearing common skin lesion/sore/tag/growth/wart may be done with liquid nitrogen (freezing) or clipped/cut off when causing irritation, redness, itching, "snagging or catching" on clothing.

                  Level 1 X-Ray

                  Cost: $102.00

                  Simple X-Ray images associated with but not limited to: adult leg, finger, foot, wrist, arm and hip.

                  *Limited availability at the UrgentCare location. Patients may be referred to Livingston HealthCare for imaging services.

                  Level 2 X-Ray

                  Cost: $146.00

                  More complex X-Ray images associated with but not limited to: elbow, knees, ankles and toes.

                  *Limited availability at the UrgentCare location. Patients may be referred to Livingston HealthCare for imaging services.

                  Level 3 X-Ray

                  Cost: $194.00

                  Complex X-Ray images associated with but not limited to: abdomen, feet, spine, or may include more views

                • Occult Blood

                  Cost: $16.95

                  Blood that is present in amounts too small to be seen and can be detected only by chemical analysis or microscopic examination.

                • Basic Metabolic Panel

                  Cost: $49.00

                  A blood test that measures your sugar (glucose) level, electrolyte and fluid balance, hematocrit and hemoglobin, and kidney function.

                • Lab Draw

                  Cost: $13.00

                  Fee charged for the collection of the lab sample

                • Influenza A & B Rapid Screening Test

                  Cost: $80.00

                  Used to help determine whether or not a person may have Influenza A or B.

                • Mono Spot Rapid Screening Test

                  Cost: $18.00

                  Used to help determine whether or not a person may have Infectious Mononucleosis.

                • Pregnancy Urine Test

                  Cost: $25.00

                  Used to help determine if someone may be pregnant.

                • Strep Rapid Screen Test

                  Cost: $56.00

                  A laboratory test done to identify germs that may cause infection in the throat.

                • Urinalysis Dipstick

                  Cost: $21.00

                  A basic diagnostic tool used to determine pathological changes in a patient's urine in standard urinalysis.

                • Fingerstick Glucose

                  Cost: $18.00

                  A procedure in which a finger is pricked with a lancet to obtain a small quantity of capillary blood for testing.

                • Preventive Screenings & Physical Exams

                  Blood Pressure

                  Cost: No Charge

                  Manual or automatic(machine) blood pressure checks are done as a walk in (appointment not required).

                • Diabetes/Blood Sugar

                  Cost: $18.00

                  Random Glucose (blood sugar) level finger stick.

                • TB Skin Test (PPD) Placement

                  Cost: $31.00

                  A person given the tuberculin skin test must return within 48 to 72 hours to have a trained health care worker look for a reaction on the arm.

                • Urinalysis

                  Cost: $21.00

                  Used to diagnose Urinary Tract Infections and other urinary tract conditions.

                  Camp Physicals

                  Cost: $94.00

                  Usually a camp physical is a limited exam of a normally healthy person/child with specific camp form needed to attend a camp. DISCLAIMER - your normal routine well check up should still be done with your primary care provider and children should maintain well child care with their pediatric/family physician.

                  Sports Physicals

                  Cost: $94.00

                  Usually a sports physical is a limited exam of a normally healthy person/child with specific state requirements to engage in sports. NOTE: filling out the history and vaccine status on the front page of the Sports Physical form will be much appreciated (sports physical for out of Montana State should provide the appropriate form needed). DISCLAIMER - your normal routine well check up should still be done with your primary care provider and children should maintain well child care with their pediatric/family care physician.

                  Travel Physical Exams

                  Cost: $94.00

                  Usually a limited physical exam of a normally healthy person/child who is planning on traveling to a country outside of the United States and may require a prescription for treatment of diseases such as malaria. DISCLAIMER - your normal routine well check up should still be done with your primary care provider and children should maintain well child care with their pediatric/family physician.

                • Vaccinations

                  Flu - Seasonal

                  Cost: $36

                  Influenza vaccines for the current season will be provided.

                • Flu - Seasonal Internasal

                  Cost: $45

                  Intranasal influenza vaccine (Flumist) for current season available for patients 2-49 years old.

                • Flu - High Dose

                  Cost: $76

                  Influenza vaccines for the current season for patients over 65 years old.

                • Dtap (Infarix)

                  Cost: $60.00

                  A vaccine that helps children younger than age 7 develop immunity to three deadly diseases caused by bacteria: diphtheria, tetanus, and whooping cough (pertussis). Dtap is a booster immunization given at age 11 that offers continued protection from those diseases for adolescents and adults.

                • Tdap (Diphtheria, Tetnas, Pertussis)

                  Cost: $60.00

                  Tetanus, diphtheria, and pertussis can be very serious diseases. These three diseases are all caused by bacteria. Diphtheria and pertussis are spread from person to person. Tetanus enters the body through cuts, scratches, or wounds.

                • Injection Administration
