DECIDE is a diabetes behavior‑change and problem‑solving program designed to empower individuals to take an active role in their health. Through peer support and shared learning, participants build confidence, strengthen self-management skills, and make informed decisions to improve overall lifestyle and well being. Open to individuals 18 and older with Type I or Type II diabetes. 

In this program you’ll learn to:

• Identify and problem‑solve barriers that get in the way of healthy habits
• Understand and interpret lab numbers
• Create behavior changes that last
• Manage diabetes alongside other chronic conditions
