If you wish to receive a COVID vaccine, please call 406.222.3541 to set
up an appointment.
Please note: Livingston HealthCare is only offereing the Moderna vaccine
at this time. Please check local pharmacies or your health department

Moderna Vaccine

6 months to 11 years old
- Initial two-dose series; 4-8 weeks between doses
- No booster dose approved at this time

12 years to 17 years old
- Initial two-dose series; 4-8 weeks between doses
- Booster dose of Pfizer-Biontech Bivalent COVID vaccine (not available at
LHC) 2 months after completion of initial series or after last booster
dose received

18 years and older
- Initial two-dose series; 4-8 weeks between doses
- Booster dose of Moderna or Pfizer-BioNTech Bivalent COVID vaccine 2 months
after completion of initial series or after last booster dose received
Pfizer-BioNTech Vaccine (not available at Livingston HealthCare)

6 months to 4 years old
- Initial three-dose series; dose two is 3-8 weeks after first dose and dose
three is 8 weeks after second dose
- No booster dose approved at this time

5 years to 11 years old
- Initial two-dose series; 3-8 weeks between doses
- Booster dose of Pfizer-Biontech COVID vaccine for 5 to 11 years old at
least 5 months after completion of initial series

12 years to 17 years older
- Initial two-dose series; 3-8 weeks between doses
- Booster dose of Pfizer-BioNTech Bivalent COVID vaccine 2 months after completion
of initial series or after last booster dose received

- Initial two-dose series; 3-8 weeks between doses
- Booster dose of Pfizer-BioNTech or Moderna Bivalent COVID vaccine 2 months
after completion of initial series or after last booster dose received
Johnson & Johnson’s Janssen Vaccine (not available at Livingston
HealthCare)

- Initial single dose series
- Booster dose of Moderna or Pfizer-BioNTech Bivalent COVID vaccine 2 months
after completion of initial series

Definitions:
Additional dose: A subsequent dose of vaccine administered to people who were less likely
to mount a protective immune response after initial vaccination. People
who are moderately or severely immunocompromised and who received Janssen
COVID-19 Vaccine for their primary series should receive an additional dose.
Booster dose: A subsequent dose of vaccine administered to enhance or restore protection
which might have waned over time after primary series vaccination.
mRNA vaccine: Pfizer or Moderna vaccine

Notice regarding the COVID vaccine cost:
The COVID vaccine is free of charge, however a small administration charge
is associated. Livingston HealthCare will submit the charge to your insurance,
and you can expect to receive a statement stating “insurance pending”
until insurance processes the claim. If you are registered with no insurance
coverage, you may expect to receive billing statements while we are verifying
and confirming any insurance coverages, however at no point will any patient be responsible to pay any charges associated
with the COVID vaccine. If you have questions, call 406.823.6414.
There are many benefits to getting vaccinated for COVID-19 and doing so
will help end the pandemic.
We still need to wear masks, practice physical distancing and wash our
hands to prevent the spread of illness. Livingston HealthCare will provide updates as they are available, and
we recommend watching for additional details from state and local public
